About Centrecare

Centrecare is a not-for-profit organisation with over 300 employees and is committed to delivering quality professional counselling, support, accommodation, mediation and training services in over 11 locations across the Perth metropolitan, Goldfields and South West regions.

The Service

The program provides psychosocial support to consumers and carers, including those diagnosed with a mental illness, who are accessing personalised support linked to Stellar Living housing. Support is delivered through a range of oneonone activities with the primary consumer, usually within their home or preferred environment.

The Role 

This position will provide practical, home-based assistance to develop daily living and independent living skills in addition to delivering individual psychosocial support and facilitate access to mental health, physical health, and community services.

A few specifics:

  • Assist consumers to build or reestablish family, social, and community networks.
  • Support engagement in recreational, educational, training, and volunteer activities aligned with consumer goals.
  • Provide advocacy, information, and referrals to promote wellness and recovery.
  • Assist consumers to maintain suitable housing, including support with tenancy issues.
  • Liaise with Stellar Living to support consumer goals and housing related needs.
  • Support carers to participate in community and wellness focused activities.
  • Refer carers to respite and community support services.
  • Cofacilitate carer peer support groups, education, and training to promote self-care and wellbeing.
  • Assist carers to access respite options.

Selection Criteria

  • A minimum of a Diploma in the field of Mental Health, Social Work, Community Services, Counselling or equivalent
  • A minimum of 1-2 years’ experience in a mental health related service
  • Demonstrated experience in a mental health related service
  • Demonstrated knowledge and understanding around case managing vulnerable, disadvantaged and complex needs clients
  • Experience and/or understanding of working with remote Aboriginal communities and their unique needs
  • An understanding of working within a stepped care approach to mental health
  • Ability to communicate and network with a range of stakeholders.
  • Computer literate with the ability to work with a range of databases
