About the Service

The Family Support Network (FSN) is a service delivery model funded by the Department of Communities and is offered throughout the Perth Metropolitan area. The service prioritises Aboriginal families and children, and Centrecare delivers this service in partnership with Wungening Aboriginal Corporation to provide an integrated approach.

  • The Family Support Network is designed to assist families with:
  • improving parenting skills to safely care for their children at home,
  • receive an integrated and coordinated service response to divert them from the child protection system,
  • develop and strengthen culturally safe and support networks.

The Role

Provide direct client intake, assessment, case plan development and review for children, young people (up to the age of 25) and families who require secondary support assistance and interventions.

  • Act as a part of a triage service facilitating initial screening, assessment and allocations for all people contacting the service via the telephone service centre leading to immediate referral or provision of information.
  • Undertake initial face-to-face screenings for clients who walk in without having used the telephone service as a first port of call.
  • Assist with the provision of interim short-term active holding management for those who are temporarily unable to access additional support services in the community.
  • Support the client through the assessment and referral processes to the point where they are linked to an appropriate service.
  • Support clients through the case plan development, review, and closure to ensure the child, young person or family received the appropriate level of support and identify if additional support services could further assist the family.
  • Work with the Leader Child Protection when there are significant child protection concerns for families to develop and establish a case plan to reduce child protection risks
  • Regularly liaise with FSN partner agencies and local community-based support services to provide warm referrals for families to access appropriate services.

Selection Criteria

  • Recognised Diploma level qualifications in Community Services, Social Work, Psychology, Youth Work and/or related disciplines
  • Demonstrated experience working collaboratively with other organisations to achieve positive community outcomes
  • Demonstrated cultural awareness and understanding of Aboriginal and Torres Strait Islander history
  • Understanding and knowledge of working with families and children including with Aboriginal and Torres Strait Islander families and children
  • Understanding of relevant legislation, regulations and external standards that apply to relevant work practices
  • High standard of assessment skills
  • Highly proficient verbal and written communication skills
  • Solution focused approach to problem solving
  • Ability to be an effective team member
  • High attention to detail
